NASHnext® Advanced Liver Risk Test

A closer look at liver health.

Your liver health can change over time, and symptoms may not be noticeable. This test is designed to look for patterns that may be associated with metabolic-related liver risk for people who may be at higher risk for metabolic related fatty liver conditions, such as Metabolic Dysfunction-Associated Steatohepatitis (MASH), which is often linked to factors like type 2 diabetes or abnormal metabolic health markers.

 

The NASHnext® Advanced Liver Risk Test measures a combination of blood based biomarkers associated with fat buildup in the liver, inflammation, and liver scarring (fibrosis). These markers are analyzed together using an advanced algorithm to estimate the likelihood of metabolic related liver conditions such as MASH, previously called nonalcoholic steatohepatitis (NASH) and moderate to advanced fibrosis.


Unlike standard liver enzyme tests like ALT (Alanine Aminotransferase) and AST (Aspartate Aminotransferase)—which measure a limited set of markers and may be within normal range even when liver changes are present—this advanced blood test looks at multiple markers. 


Note: Results are not diagnostic and cannot tell whether you have liver disease. Instead, they are intended to help support follow-up discussions with a healthcare provider. 


This test is not intended for people whose liver concerns are likely caused by other conditions, including viral hepatitis, alcohol related liver disease, medication related liver injury, genetic or autoimmune liver diseases, or certain bile duct disorders.

Unlike routine liver enzyme tests alone, this approach looks at how several markers relate to each other. Results are intended to support clinical follow-up and monitoring decisions when interpreted by a healthcare provider and are only one part of a broader liver health evaluation.

  • The Labcorp OnDemand Liver Health Test focuses on liver function by measuring enzymes and proteins like ALT, AST, bilirubin, and albumin and includes a FIB‑4 score to screen for fibrosis risk. NASHnext® is more specialized; it uses advanced biomarkers and a proprietary algorithm to estimate risk specifically related to MASH and moderate to advanced fibrosis. Both tests offer valuable insight. NASHnext® is designed for a deeper look at metabolic‑related liver risk.

  • A NASH profile blood test looks at multiple blood‑biomarkers together to estimate risk for metabolic dysfunction‑associated steatohepatitis and liver fibrosis. The NASHnext® test uses a validated algorithm (NIS4) that combines markers associated with liver inflammation, metabolic health, and scarring into a single risk score. This approach provides a more nuanced picture of liver health and risk for advanced liver complications compared to routine liver enzyme tests.

    While this test can provide useful information, it is not diagnostic and cannot confirm or rule out NASH/MASH. Imaging tests or other evaluations may still be recommended by a healthcare provider depending on your risk level and medical history.

  • The NASHnext™ score combines information from four blood markers into a single number that helps estimate liver‑related risk. Each marker gives a different piece of information about liver health:
     

    • miR34a50 reflects changes in liver cells that can occur with liver stress, inflammation, and scarring. It is linked to how liver cells manage fat and respond to injury.
    • YKL40 is linked to immune cell activity and may rise when there is ongoing liver inflammation or scarring.
    • Alpha2 macroglobulin is a protein that can increase when the body is responding to liver scarring over time.
    • Hemoglobin A1c (HbA1c) shows average blood sugar levels over the past few months. Higher levels are associated with metabolic stress, which can affect liver health and inflammation.


    Together, these markers are analyzed to place results into risk categories, offering context about your liver health. The score does not diagnose liver disease and should be discussed with a healthcare provider as part of a full health evaluation.

  • Many people never develop noticeable symptoms, which is why fatty liver disease is often found during testing done for other reasons.

  • There isn’t a single blood test that definitively confirms NASH/MASH. Tests like the NASHnext® Advanced Liver Risk Test combine multiple specialized biomarkers into a risk score, offering more advanced insights about your liver health.


    This test does not provide a diagnosis but can help identify people who may be at lower or higher risk and support next step conversations with a healthcare provider.

  • Not necessarily. Elevated ALT and AST can be linked to fatty liver, but they can also rise due to other factors or remain normal despite underlying liver changes. That’s why more advanced tests look beyond enzymes to provide additional insight into fatty liver and fibrosis risk.

  • Yes. Many people with early or even moderate fibrosis have normal routine liver test results. Advanced blood‑based assessments can sometimes detect fibrosis related patterns before enzyme levels change, helping identify risk earlier.

  • Simple fatty liver involves fat buildup without significant inflammation. NASH/MASH includes inflammation and liver cell injury, which increases the risk of fibrosis over time. Identifying risk earlier can help guide monitoring and lifestyle discussions.

  • Your testing will be performed in a CLIA-certified clinical laboratory. Labcorp provides leading-edge medical laboratory tests and services through a national network of primary clinical laboratories and specialty testing laboratories. Our labs perform regular quality assurance testing to ensure our results are accurate.

  • Your results will be provided to you through our secure online portal to view or print. Results are also made available through a secure portal to the independent healthcare provider who placed your test order.  If you are accessing your results through your Labcorp Patient™ account, all users linked to the account will have access to all OnDemand test results. Results may also be provided to a local, state, and/or federal health authority; for example, many states require the testing laboratory and physician to report test results for certain communicable diseases, such as COVID-19, to local or state health departments.
