The thyroid peroxidase (TPO) antibody test measures the level of thyroid peroxidase antibodies in your blood. The immune system produces these antibodies if it mistakenly attacks the thyroid gland. This can lead to autoimmune thyroid disorders and other conditions. Elevated TPO antibodies are more commonly associated with Hashimoto’s thyroiditis, which causes hypothyroidism (an underactive thyroid), but they can also be present in Graves’ disease, which leads to hyperthyroidism (an overactive thyroid). These conditions can cause symptoms such as fatigue, weight changes, hair loss, irregular menstrual periods and mood changes. Some individuals without evidence of thyroid disease also have elevated TPO antibodies in their blood.

 

Testing for TPO antibodies can help you and your healthcare provider evaluate symptoms and conditions that may be related to an elevated TPO antibody level. TPO antibody is often evaluated in the context of other thyroid function tests such as thyroid-stimulating hormone (TSH), free T4 and free T3 to gain a full picture of your thyroid health.

 

Note: A TPO antibody test alone cannot assess how thyroid function.

    Thyroid peroxidase (TPO) antibody is a protein produced by the immune system that targets thyroid peroxidase, an enzyme needed for thyroid hormone production. This test measures the level of TPO antibodies in the blood. Elevated TPO antibody levels are often associated with autoimmune disorders such as Hashimoto’s thyroiditis and Graves’ disease but can be related to other conditions. These antibodies can indicate an ongoing immune response against the thyroid, potentially leading to inflammation and impaired thyroid function. Elevated TPO antibody levels may contribute to symptoms such as fatigue, weight gain, mood changes and irregular menstrual periods if thyroid function is abnormal. Some individuals will have elevated TPO antibodies without any evidence of a thyroid disorder.

High TPO antibody levels may also indicate ongoing inflammation or damage to the thyroid, which may lead to hypothyroidism or hyperthyroidism. However, TPO antibody levels alone do not determine thyroid function. Thyroid function tests, such as TSH and free T4 are needed to assess thyroid activity.
